โดยหนังสือฉบับนี้ ข้าพเจ้าขอให้ความยินยอมแก่สถาบันเพิ่มผลผลิตแห่งชาติในการเก็บรวบรวม ใช้ และเปิดเผยข้อมูลส่วนบุคคลของข้าพเจ้าที่ให้ไว้แก่สถาบันเพื่อวัตถุประสงค์ในการประชาสัมพันธ์ข่าวสารและการตลาด เช่น การแจ้งข่าวสารด้านการเพิ่มผลิตภาพ การนำเสนอผลิตภัณฑ์และบริการ ข้อมูลทางการตลาด และกิจกรรมส่งเสริมการตลาด ดังนี้
